Expertly Trained Staff
Dementia care is specialized care. Each of our residents are on a unique journey and it takes a deep understanding of dementia to provide effective care. Our entire staff, not just our caregivers, completes a comprehensive and interactive 32-hour CARES dementia training program when they join the Grand Brook family. This training provides a thorough background in understanding the progression of dementia as well as effective ways to connect with and care for our residents. Being able to recognize each resident’s needs as wholly unique allows us to provide truly resident-centered care.
Individualized Care Plans Thorough Assessments
Every Grand Brook resident receives a unique care plan designed around them. What exactly is a care plan? It’s an individualized blueprint developed in collaboration with your family and the entire Grand Brook care team to better understand your loved one’s unique needs and preferences.
These individualized plans begin with a comprehensive assessment. A thorough assessments is completed by the Grand Brook nursing team prior to move-in, and then again within the first two weeks of residency, annually, and whenever a change in health status is noted. Care plans are reviewed regularly during family care conferences.
Memory Healthcare That Goes the Extra Mile
For families requiring the relocation of a loved one residing out of town or state, our nursing team will meet with the family and potential resident to determine if a Grand Brook community would be a good fit. Once the decision has been made to join a Grand Brook community, our team will caringly escort your loved one to our community at no additional charge. Whether by plane, train, or automobile, our care team will go the extra mile. We are always here to help ensure the safety of your loved one as they relocate from your home, a hospital or another residential community.
