Dementia care is specialized care, and each of our residents is on a unique journey. It takes a deep understanding of dementia to provide the best care, and our entire staff—beyond just our caregivers—completes a comprehensive 32-hour CARES dementia training program when they join the Grand Brook family. This training equips our team with a thorough understanding of dementia’s progression and effective ways to connect with and care for our residents. By recognizing each resident’s unique needs, we’re able to provide truly resident-centered care. To ensure our team stays knowledgeable and up to date on best practices, all staff participate in ongoing dementia education throughout their journey with Grand Brook.
Individualized Care Plans & Thorough Assessments
Every Grand Brook resident receives a personalized care plan tailored to their unique needs. But what exactly is a care plan? It’s a detailed blueprint developed in collaboration with your family and the Grand Brook care team to ensure we understand your loved one’s specific needs and preferences.
These individualized plans begin with a comprehensive assessment. Our nursing team conducts a thorough evaluation before move-in, again within the first two weeks of residency, annually, and anytime a change in health status is noted. Care plans are also reviewed regularly during family care conferences to ensure they continue meeting your loved one’s needs.
Memory Healthcare That Goes the Extra Mile
For families needing to relocate a loved one from out of town or state, our nursing team will meet with the family and potential resident to determine if a Grand Brook community is the right fit. Once the decision is made, our team will compassionately escort your loved one to our community at no additional charge. Whether by plane, train, or automobile, our care team truly goes the extra mile. We are always here to help ensure a smooth and safe transition—whether from home, a hospital, or another residential community.
