Jane Anderson is the Founder of Grand Brook Memory Care. A registered nurse and dedicated eldercare advocate, Jane has worked in the healthcare industry for more than 30 years and has specialized in memory care and neurological impairment for 20 plus years. Her dedication to individuals with memory impairment and their families is the reason that our Grand Brook communities exist today. She continues to be involved in the daily operations at each community and provides guidance to each location ensuring that they stay true to her vision of providing dedicated, quality, home-like memory care to each resident.
Prior to starting the Grand Brook Memory Care communities, Jane’s involvement with the launching of the first off-site cancer center in Lansing, Michigan was paramount to its success (it is still open and operating today). Jane’s warmth and compassion for helping people took her around the northern states administering pain management, anti-virals and IV medication to individuals living with HIV/AIDS throughout the stages of the disease. She has received multiple degrees and certifications including her Bachelors of Science in Nursing from Michigan State University, Oncology Nursing Certification from University of Michigan, Registered Nurse Infusionist Certification and a Hematology Certification from Wayne State University.
In Jane’s free time she enjoys volunteering with CASA, homeless shelters, the Alzheimer’s Association and spending time with her children and grandchildren.
Chad is the President of Constant Care Management Company, and oversees the day-to-day operations of Grand Brook Memory Care communities.
Chad manages the Executive Directors of Constant Care’s Grand Brook Memory Care branded residences, and establishes operating policies and procedures across the company with a team approach and focus on mutual respect and accountability. He is responsible for the development, delivery and ongoing management of a wide variety of services and functions across Grand Brook Memory Care. Chad is responsible for resident Life Engagement programs, culinary operations, operating budgets, customer relations, facility management and development, marketing, staffing, and human resources. In addition, he manages the Quality of Resident Care Program which includes continual on-site inspections, employment training, and more; ensuring that best practices are carried out and the well-being of the residents come first.
Chad’s twenty years of direct, hands-on experience and commitment to operational excellence are invaluable and help to facilitate Grand Brook Memory Care’s “Generations of Caring” culture and environment.
Chad holds an Executive Master’s degree in Business Administration from Southern Methodist University and a Bachelor of Arts degree in Business Administration-Marketing from Western Michigan University. He and his wife, Anisa, reside in North Texas with their three children. Chad and Anisa are very active in the Alzheimer’s Association and give their time generously to the community.

Jamie Lopez is Vice President of Healthcare at Constant Care Management and oversees the Nursing and Healthcare Division of the company. Jamie joined the company in 2003, as a community based Executive Director. In 2008 she assumed her role as Vice President of Healthcare.
As Vice President of Healthcare, Jamie is responsible for creating and monitoring all healthcare and nursing policies and procedures, developing and managing resident testing and assessments, establishing best practices, sharing current information through the company related to the most up to date research and advances in Alzheimer’s disease and other related dementias, and developing and conducting staff and RN training programs.
Jamie’s hands-on approach to management ensures continuity of the highest quality of care across Constant Care’s communities and embodies the company’s family-oriented philosophy of care. She personally visits communities to monitor residents, meet with families, monitors current practices and care delivery approaches, meets with the nursing team, caregivers and other local staff members. Providing regular on-going trainings are also part of Jamie’s commitment to ensuring that all staff is educated on the up-to-date care practices and advances in memory care.
Prior to joining Constant Care, Jamie completed a nursing internship at Children’s Medical Center of Dallas and worked as a clinical Registered Nurse at the University of Michigan Hospital. Dedicated to continual learning and improvement, Jamie remains up-to-date on the most recent research and recommendations for the care of aging adults and those with Alzheimer’s and other related dementias, as well as traditional medical care. She is actively involved in educating patients' families to help them better understand their loved one’s situation and establishing support groups on the aging process, Alzheimer’s and dementia. Jamie is a certified national dementia trainer for both professional and non-professional caregivers.
Jamie graduated from the University of Michigan with a Bachelor of Science Degree in Nursing, graduating summa cum laude and as a member of Sigma Theta Tau, the international nursing honor society. During her time at the University, Jamie worked as a caregiver in an assisted living residence for those with Alzheimer’s and other forms of dementia. She is currently still active with the local Alzheimer’s Association. Jamie currently lives in McKinney, Texas with her husband and three children.
Bill Anderson has been the Vice President overseeing five all inclusive memory care communities for Constant Care Management Company. As Vice President, his responsibilities include: directing the organization’s operational activities for the DFW East Region, hiring and leading the management team for each community, and evaluating the efficacy of the current policies and procedures.
Bill joined Constant Care Management Company in 2008 and served as an Executive Director for two years. In 2010, he was promoted to his current position bringing with him extensive knowledge and experience of Alzheimer’s and dementia, operating procedures, and the interpersonal skills required to manage both. Bill relies on his knowledge and experience to positively impact residents, their families, and the community’s staff. Furthermore, he believes that a community’s long term success is achieved through a combination of financial understanding and a wide degree of creativity, and strong interpersonal skills.
As a family man with a strong desire to help others Bill focuses on family satisfaction and staff development to provide a healthy workplace and a loving home. Helping others started at an early age and was a motivating factor in his earning of a Bachelor’s Degree in Education. For Bill there is a certain satisfaction being able to work with people and assist families and their loved ones at all ages. He looks forward to helping to develop Grand Brook Memory Care - a place many will call home in the future. In his spare time Bill cherishes the time spent with his wife and two children. He also enjoys coaching youth sports and serves on the little league board of directors in his home community.
Jennifer Plunkett is the Vice President of Asset Preservation for Constant Care Management. Jennifer joined the company in 2004 as the Director of Design to manage the design of new construction including interior and exterior design as well as landscape planning. Jennifer became very involved in the construction process and enjoyed overseeing and carrying through design standards. She was responsible for ensuring the company was utilizing best practices to create a community specifically to address the needs of those with memory loss. “It brings me so much joy to be working in a field where I can create the best possible environment for seniors with dementia. I love the challenges and creativity in designing for these individuals to create a place our residents call home.”
After much success in the Director of Design position, Jennifer was promoted to Director of Capital Improvements. In this role Jennifer was responsible for managing all capital improvements at the communities and managing capital budgets for all properties in operation. Jennifer toured and communicated with staff regarding which design aspects to incorporate to create a beautiful, comfortable and safe home for the residents.
Additionally Jennifer’s responsibilities included performing extensive research and studying the latest techniques and design elements available to create an environment with a home-like feel conducive to the specific needs of those living with memory impairment.
Prior to joining Constant Care Management, Jennifer received her Bachelor’s Degree in December of 2003 from Western Michigan University. She moved to Texas to work alongside her family to share her knowledge and to help expand CCMC to help more seniors in need of memory care services. She and her husband currently live in McKinney, Texas with their three children.
Candice Montoya is the Controller and Treasurer for Constant Care Management. She graduated from the University of North Texas with a Bachelor of Accounting degree and from Southern Methodist University with a Master of Science degree in Accounting. She is a licensed Accountant and a member of several professional organizations for Accounting & Healthcare.
As Controller, and Treasurer, Candice is responsible for overseeing the financial reporting, cash management and daily accounting/payroll functions for the company. Candice’s responsibilities directly support the operations by managing all community business functions allowing our community team to spend their time working with residents & families.
From her prior work experience as a Senior Auditor, Candice learned the necessity of implementing processes and controls to protect the integrity of financial information, and she brings this unique perspective to her position as Controller to maintain the company’s fiduciary duty and serving as a liaison between the communities and their investors to promote successful operations. She is responsible for continually updating current control measures and processes in order to not only protect the current properties managed by Constant Care but also to sustain future financial growth.
